If you are under the age of 24, then when you apply, we will need your parent(s) 1040 tax form. If you do not have access to their form, you must fill out the income eligibility form with your parent(s) signature. 

If you are at or above the age of 24, then you will need to provide your own 1040 tax form.

If you have trouble accessing your 1040, we also accept a SAR.

You can request the income eligibility form via email.

When applying for our program, you must send us your most recent transcipt, which can be unofficial.

To access your unofficial transcript, follow the steps below:

1. Go to your myWSU portal
2. Click on the myClasses tab
3. Under the Academic Profile section right below your listed advisor should be a link to your most recent transcript.

Another part of your application that we will need are your VARK Questionnaire results. You can send us a pdf print out of your results or a screenshot. Go to to take the quiz.
